Things might feel difficult or tough at the moment, but by choosing to provide your contact details, we'll be able to help provide you with better support and to stop something similar from happening to anyone else. The information you provide will only be accessed by key members of staff. Your confidentiality will be respected and information will not be shared without your consent unless you or others are considered to be at risk.
When a record is submitted you will also be given a reference number which, should you choose, can be shared with or referred to when talking with staff about your case. Once you have submitted a record, you cannot withdraw any information.
